Life Insurance and Disability

You cannot predict the future, but you can plan for it. The Plan includes more than just health care benefits—we offer income protection in the event of an illness or injury as well as benefits for your family in the event of your death. In fact, the Five-Year Family Protection Benefit gives your family five years of free in-network health care coverage should you die during active employment.

If you die while working full-time, your designated beneficiary is eligible for a Life Insurance benefit equal to 2080 X Your Hourly Rate in effect at the time of your death. So, if your hourly rate is $30.00, your beneficiary would receive a lump-sum payment of $62,400.

If you work part-time, your beneficiary is eligible for a benefit equal to 1040 X Your Hourly Rate in effect at the time of your death or $40,000.00,whichever is greater. So, if your hourly rate is $15.00, your beneficiary would receive a lump-sum payment of $40,000.  If your hourly rate is $39.00, your beneficiary would receive a lump-sum payment of $40,560.00

NOTE: The numbers used as hourly rates above are only examples. Actual payments depend on actual hourly rates.

 

IS YOUR BENEFICIARY UP-TO-DATE?

In the event of your death and if you have not named a beneficiary or if the beneficiary you named is no longer living, the benefit amount will be paid in full to the first surviving class as follows:

  • Your surviving spouse
  • Equal shares to your surviving children;
  • Equal shares to your surviving parents;
  • Equal shares to your surviving brothers and sisters; or
  • Your estate.

 

Spouse Life Insurance

You are eligible for a $5,000 Life Insurance benefit if your covered spouse dies while you are a participant in the Plan. The member is always his or her spouse’s beneficiary.

 

Child Life Insurance

You are eligible for a $2,500 Life Insurance benefit if your covered child dependent dies while you are a participant in the Plan.

 

Accidental Death and Dismemberment (AD&D)

If you work full-time and die as a result of an accident, the Plan pays your beneficiary a benefit equal to 2080 X Your Hourly Rate. If you work part-time, your beneficiary is eligible for a benefit equal to 1040 X Your Hourly Rate. AD&D benefits are payable up to a maximum of $100,000.

 

Short-Term Disability

If you become sick or injured and cannot work, the Plan will replace a portion of your income during your recovery. The Short-Term Disability benefit pays 60% of your average weekly pay up to $500 per week for a maximum of 26 weeks. Long-Term Disability continues to be provided by UPS and kicks in after 26 weeks.

 

Five-Year Family Protection Benefit

For active Teamsters, the Plan offers an extraordinary benefit that was not offered by your former UPS Company Health Care Plan—The Five-Year Family Protection Plan.

 

In event of your death, your family is eligible for five years of continued health care coverage at no extra cost—no COBRA; no premiums—just the same out-of-pocket costs they would pay if you were still an active member. With family health insurance on the open market costing about $25,000 each year, this benefit offers considerable savings— and peace of mind.

 

To be eligible for this special benefit, families must have been using in-network medical providers for all non-emergency medical care during the two-year period prior to your death.
