Did you know that you may be eligible for Health benefits even when you are unable to work due to illness or injury?  If you are injured on the job, your employer may contribute to the plan for up to 52 weeks of eligibility.  If you have been approved for a Family Medical Leave, your employer may contribute to the plan for up to 12 weeks of eligibility.  If you are disabled due to a non-work related illness or injury, you may be eligible for Short Term Disability benefits.  If approved for Short-Term Disability, your benefits will be extended with a disability waiver up to 26 weeks.  If your disability continues beyond 26 weeks, and you are a Full-time employee, you may be eligible for Long Term Disability (LTD).  Your employer may contribute up to 52 weeks if approved for Long Term Disability.  If your disability continues beyond 26 weeks and you are a Part-time employee, you will be offered the opportunity to continue coverage through COBRA (Consolidated Omnibus Reconciliation Act).

 

If disabled from work, you must notify UPS HRSC at (855) 877-4772 and complete the appropriate documents with your local HR.  If you work in NJ, NY or HI, you will need to file a disability claim with Aetna at www.wkabsystem.com or you may call 1-800-877-1508 to reach an Aetna Disability Representative.  If you reside in CA, you will need to file a claim with both the State of California and with the Trust Fund.   For California benefits, you may go to  http://www.edd.ca.gov/Disability/Benefits.htm.  For Trust Fund benefits, Disability Filing Procedures and Forms are available on this website.  You may also contact the Fund Office at (855) 215-2039 for additional assistance with your Disability claim.

 

For LTD Benefits, you must contact Aetna at (866) 825-0186 with sixty (60) days advance notice (around your 17th week of Disability) in order to initiate a LTD claim.
