If you have not yet received an important packet from the Health Plan containing a Summary of Material Modifications and other plan compliance information, please refer to the Mailings posted on this website and contact the Fund Office at (855) 215-2039 to verify we have your correct address.

If you reside in CA and HI, you may also change your Medical Carrier during Open Enrollment which ends on 11/16/2017 for Medical Carrier changes to be effective 1/1/2018. If you wish to change your Medical Carrier, you may download an Election Form from this website. All Open Enrollment changes must be received at the Fund Office by 11/16/2017.
